Insurance

Georgialina Physical Therapy Associates accepts all insurance except Medicaid. As a courtesy for our patients we will contact your insurance carrier at the time of your initial visit to determine your benefits for physical therapy. Additionally, we encourage our patients to also contact their insurance carrier to be aware of their physical therapy benefits regarding any limitations of coverage as well as co-pay, co-insurance and deductible information.

We offer direct insurance billing and are a network provider for all major health insurance companies to include, but not limited to, the following:

• Blue Cross Blue Shield
• Tricare
• Medicare
• Cigna
• Aetna
• United Health Care

We also provide care and billing for workers compensation and automobile accident claims.

Patients are responsible for:

• Insurance deductible
• Co-insurance
• Co-payments per office visit

At the time of your first visit please have:

• Current insurance card.
• Picture I.D.
• Prescription for physical therapy from a referring doctor.

We understand you may have questions regarding billing and payment options. We will be happy to discuss a payment plan for you. We accept the following methods of payment:

• Cash
